| Playground Inspection Worksheets Paper Versions of the worksheets found on this page can be printed out and filled in by hand. Copies of the paper versions of my worksheets are also available in Appendix A of the fourth Edition of PLAYGROUND SAFETY IS NO ACCIDENT by Kenneth S Kutska. Playground Safety is No Accident has been given to every student who has taken the National Playground Safety Institute (NPSI) Certified Playground Safety Inspector (CPSI ) course. NPSI is no longer including the book in the instructional materials given to students but they are providing a CD of electronic fill versions of the forms Ken uses in the book. The book can be purchased directly from Ken at www.internationalplaygroundsafetyinstitute.com. For a sample of the forms click here for additional information or to place an order, please contact david@spease.com The forms can be saved to your computer as a library then used to create print and store reports for your projects The set includes a set of worksheets for conducting Audits for play areas for children 2 years through 12 years of age (ASTM F 1487) and a separate set for play areas for children 6 months through 23 months of age (ASTM F 2373) Each play area inspection should include one of each of the the following general worksheets; I recommend grouping them in the order they are listed. Each age appropriate grouping of components should be treated as a separate inspection, even if two age appropriate groupings are in the same play area enclosure. Each playground inspection report should have a summary report that identifies all areas of concern and discusses potential repair, modification or replacement options. The worksheets are in PDF format and can be opened with Adobe Reader. If you do not have Adobe Reader it is a free download on the Adobe web page at www.acrobat.com Save the worksheets to your computer to use as templates for your Playground Safety Inspections. THESE FORMS ARE DESIGNED TO WORK WITH ASTM F 1487-07ae1, ASTM F 2373-08 and CPSC Handbook #325. USE OF THESE FORMS DOES NOT GUARANTEE AN ACCURATE INSPECTION THEY ARE SAMPLES ONLY TO BE USED AT YOUR OWN RISK AND MUST BE SIGNED BY A CERTIFIED PLAYGROUND SAFETY INSPECTOR TO BE VALID. Executive Summary Samples These Executive Summaries are given as examples only. They may be modified to meet your needs as an inspector based on the types of services that you provide etc. and the needs of your clients. If a series of inspections for multiple sites is required, you may wish to provide a spread sheet summary for the project in addition to a summary sheet. The work sheets are used to assure a complete report but most clients will want to have a simple summary. They can read the complete worksheets to learn specific information about the requirements. Summary With Approval Provide your client with an Executive Summary similar to this sample on your letterhead with your reports for play areas that meet the requirements. Summary With Concerns Provide your client an Executive Summary similar to this sample that outlines conflicts and indicates options for modifications that may be available.

Worksheets - 6 Months Through 23 Months User Group This age group is primarily for child care facilities that provide infant and toddler services but it is occasionally found in High Schools that teach child care as part of the curriculum or to provide child care for students that may have children. Some Parks Agencies also lease park property to child care facilities that may have this type of playground.
Worksheets - 2-12 Year Old User Group This it the typical playground that we are all familiar with in Parks, Schools and other public use playgrounds.
